Complete your transaction then, reply to the processing email you receive and attach a file with the delivery addresses.

Best Excel format would be separate columns with:

FIRST NAME  *  LAST NAME  *  ADDRESS1  *  ADDRESS2  *  CITY  *  STATE  *  ZIP  *  GIFT * SPECIAL MESSAGE
